Chronic Condition Support

Acuity Homecare offers support for the person with chronic conditions as an alternative to the status-quo. We provide caregivers who are experienced in assisting clients with advanced medical conditions such as heart failure, COPD, Parkinson’s disease, or kidney failure to name a few. Our staff collaborate with your physician, hospice agency or visiting nurse agency to ensure compliance with your care plan and ensure they remain appropriated for your condition.

Our Well-Qualified Administrative Staff

  • Work closely with members of the Aging Life Association to provide comprehensive education to our caregivers on the management of our clients’ lifestyle and needs.
  • Educate Acuity Homecare caregivers on the support needs of a person with chronic conditions such as Alzheimer’s/ dementia.
  • Formulates, implements and continuously oversee each of our client’s care plan. This enables quality, professional assistance to be provided in the comfort of our client’s own home.
  • Provide at minimum, biweekly field supervision of all care givers.

Alzheimer's | Dementia

“Alzheimer disease is the most common form of dementia, affecting more than one-third of Americans older than 85 years. It is characterized by progressive memory loss and cognitive decline” (Hazlewood, Onysko, et al, 2011, par. 1).


Some causes of Alzheimer’s disease have been linked to environmental and genetic factors, however, there is still no known cure or prophylaxis. Symptoms of Alzheimer’s vary with its level of severity, which ranges from mild (cognitive impairment; withdrawal; unable to perform ADLs), to moderate (disorientation; forgets names and combativeness), to severe (incontinence; dependency; forgets family members and loss of intelligible vocabulary).
